Your is a possessive determiner and refers to the second person pronoun you. You’re is the abbreviation of you are.

Incorrect:If your planning to take the week off, please let me know in advance.
Correct:If you’re planning to take the week off, please let me know in advance.
Incorrect:Before you bring in your vehicle for repair, tell us you’re needs and time constraints.
Correct:Before you bring in your vehicle for repair, tell us your needs and time constraints.
